Nathan Pelzer is the Co-Founder and CEO of Clinify Health, a venture-backed digital health company that works with health plans, federally qualified health centers, and community-based collaborative partners to succeed in value-based care delivery and reimbursement models. Founded in 2019, Clinify provides a digital suite of solutions that enable clinical and community-based care teams to develop and deploy population health strategies that incorporate clinical, social, and contractual goals and objectives into their practice operations. Prior to founding Clinify, Nathan was the Head of Corporate Strategy for a national primary care provider organization located in Chicago where he was responsible for coordinating strategic initiatives across markets nationally. Prior to this role Nathan spent his career in senior level positions within global healthcare advisory and finance organizations working across various aspects of the healthcare industry. Nathan has a B.S. in Finance from The University of Minnesota and an M.B.A. from the University of Chicago – Booth School of Business.
